- Description: Charles Manson was notorious for his antisocial behaviour and influence; through his charisma, he formed a group of followers known as the Manson family. Manson and his followers became involved in various criminal activities which culminated in the murder of 7 people. These murders were not committed by Manson per say, but he planned them, and incited his followers to commit them; this resulted in a trial and Manson going to prison, being given a life sentence. He continued to influence people while he served his sentence in prison. Manson died of natural causes in November 2017. The research study explores and describes the life of Manson in the form of a psychobiography. The aim of the study was to gain insight into the personality development of Charles Manson. This was achieved by applying Otto Kernberg’s Object Relations Theory to Manson’s known life experiences. Various data was collected and triangulated, using primary and secondary sources. Miles, Huberman, and Saldana’s (2013) three step procedure was used to analyse the data. The findings highlight the influence of genetics and rejection as pivotal to the unfolding of Manson’s personality, and provide insight into how his antisocial patterns unfolded. Maxwell wrote simple prose which paints an idyllic and moving picture of his time spent in his “lost paradise”, alongside his beloved animals, most notably his otters. However, in reality Maxwell was a man in conflict, seen in volatile relationships, financial recklessness, and the preference for the company of animals over people. The aim of this study was to explore Maxwell’s life to gain insight into his personality development. This was achieved by applying Otto Kernberg’s Object Relations Theory to his life experiences. In doing so the author hoped to gain an understanding of the personality dynamics that led to Maxwell’s relational difficulties and eccentricities. Data was collected by triangulating multiple sources of information on his life. Miles and Huberman’s (1994) three step approach was used to analyse data collected. These steps are: data reduction, data display, and conclusion drawing and verification. The research study found that Maxwell’s personality development was likely arrested at the third stage of Kernberg’s model. Maxwell relied primarily on immature defence mechanisms based in splitting and possessed a fragile ego structure which he protected through grandiosity. Harvey (1930-1978), was an American Gay Rights activist who became one of the first openly gay political candidates to be elected to public office in the United States in 1977, before being assassinated in 1978. The study was conducted within a postmodern Social Constructionist epistemology which applied Michael White and David Epston’s (1990) Narrative Therapy as a particular theoretical lens to understand Harvey’s lived experiences. Harvey was chosen as the research subject based on his ability to construct his public identity as an openly gay politician and activist in defiance of the contemporarily dominant heteronormative discourse, despite the danger this posed to his personal safety. His selection was made through purposive sampling which facilitated a qualitative, single case psychobiographical study. The study’s findings detail the narrative processes Harvey underwent and the discursive influences he experienced to move from being a closeted gay man, who hid his sexuality from public view, to an openly gay trailblazing Gay Rights activist, who fiercely challenged the norms and effects of heteronormativity on both his own life and that of his gay community. An educational intervention on the prevention of catheter-related bloodstream infections in haemodialysis patients in Eswatini

By maintaining an infection-free environment, it is expected that the patient's recovery will be promoted, and high-quality nursing care will be delivered. This also applies to the prevention of catheter-related bloodstream infections (CRBSIs) in haemodialysis patients. As nurses spend most of their nursing time with patients, they should have a good level of understanding of infection prevention and control in health care setups. In Eswatini, central venous catheters (CVCs) are commonly used for vascular access in patients who require haemodialysis. From the latest data available in the country's three largest regional hospitals, the proportion of haemodialysis patients with CVCs hospitalised for CRBSIs was 27% (64/239) January to December 2020. Further, it is unclear whether professional nurses have knowledge on the prevention of CRBSIs. This study aimed to investigate the effect of an educational intervention, based on the Centre for Disease Control (CDC) Guidelines, on professional nurses' knowledge regarding the prevention of CRBSIs in haemodialysis patients in hospitals in Eswatini (previously known as Swaziland). The study used a convenience sample including all professional nurses and nurse managers (N=99) employed at the renal and intensive care units (ICUs) at the five hospitals (Hospitals A-E) in Eswatini where haemolysis is conducted. The study is underpinned by Florence Nightingale's Theory and the Knowledge-to-Action Framework. The research study followed a positivistic, quantitative research paradigm, using a quasi-experimental, one-group, pre-post-test design with an interventional research approach, to be conducted in three phases. The first phase was the distribution of a pre-test questionnaire to all professional nurses and nurse managers working in renal units and those in ICUs that are also managing CVCs in order to assess their knowledge regarding the prevention of CRBSIs in haemodialysis patients. The second phase included the educational intervention, which was developed, reviewed and presented by the researcher to professional nurses. The third phase included the distribution of a post-test questionnaire to the professional nurses to assess the effect of the educational intervention on their knowledge.The pre-and post-test questionnaires were developed by the researcher, in line with the educational intervention (and informed based on the pre-test findings), reviewed by six experts and pilot tested to ascertain its validity and reliability. The researcher captured the data using Microsoft Excel and, with the assistance of a statistician, who performed statistical analyses using Cronbach's alpha, One-sample t-tests, Cohen's Kappa, Cohen's D, and Chi-square test. Ethical principles, according to the Belmont Report, including autonomy, beneficence and justice, and appropriate standards of rigour were maintained throughout the research study. In this study the overall knowledge was good with a mean score of 75.05 ±8.74 for the pre-test and 77.75±9.55 for the post-test. The knowledge scores for the factor infection prevention was higher (pre-test: 83.33±9.28 versus post-test: 85.14±8.28) compared to the factor catheter care (pre-test: 66.76±13.13 versus post-test: 70.38±14.27). The factor catheter care and age was significantly related in the pre-test with a p-value of .043 between groups. The factor infection prevention and how long practising in the unit were significantly related in the pre-test, with a p-value of .029 between groups. The factor infection prevention in the pre-test had a medium significant difference between >1 year and >5 years practicing in the unit (Scheffé p .048 and Cohen's d 0.77). The study is the first in the field of nursing in Eswatini. - Description: The successful prosecution of a sex crime case is dependent on the testimony of the victim. Due to the concealed nature of a sexual crime, it is notoriously difficult to prosecute effectively. With child victims, a number of additional obstacles present themselves when it comes to children giving evidence. Attrition is the rate at which cases do not proceed to court. Statistics in South Africa has revealed that rape has one of the lowest conviction rates of all serious crimes. This study is aimed at analysing the factors that cause attrition during the investigation stage in cases where children were victims of any form of sexual crime. In order to obtain the unscripted view of the investigation process, the qualitative study comprises of interviews with investigating officers who investigate these cases and the prosecutors who present these cases to court. The multi-disciplinary team-approach during investigation and its varying limits and challenges, were the main focus. A purposive non-probability sampling technique was utilised, focusing on the Kouga district municipality of the Eastern Cape. Through this study, the factors causing attrition were analysed and presented in such a manner so that change can be conceptualised in order to strive for the rights of children to receive justice for crimes committed against them. Key findings of the study yielded three significant factors which plague the progress of these cases. A significant finding illuminated the lack of required capacity professionals have in working and understanding children throughout the criminal justice process. A further vital finding is the complexities of inter-departmental functioning on implementation level, indicating challenges in collaboration. An integral finding is the measure of time taken to investigate cases of child victims, which the criminal process flow is delayed to its own detriment in aiming for convictions. In general it was found that services to child victims are neglected. Collaborative approaches which are already in place are to be strengthened through capacity building and collaborative knowledge integration in all sectors of professionals. In order to obtain the unscripted view of the investigation process, the qualitative study comprises of interviews with investigating officers who investigate these cases and the prosecutors who present these cases to court. The multi-disciplinary team-approach during investigation and its varying limits and challenges, were the main focus. A purposive non-probability sampling technique was utilised, focusing on the Kouga district municipality of the Eastern Cape. Through this study, the factors causing attrition were analysed and presented in such a manner so that change can be conceptualised in order to strive for the rights of children to receive justice for crimes committed against them. Key findings of the study yielded three significant factors which plague the progress of these cases. A significant finding illuminated the lack of required capacity professionals have in working and understanding children throughout the criminal justice process. A further vital finding is the complexities of inter-departmental functioning on implementation level, indicating challenges in collaboration. An integral finding is the measure of time taken to investigate cases of child victims, which the criminal process flow is delayed to its own detriment in aiming for convictions. In general it was found that services to child victims are neglected. Collaborative approaches which are already in place are to be strengthened through capacity building and collaborative knowledge integration in all sectors of professionals. Caregivers’ Knowledge, Attitude and Practices (KAP) of breastfeeding and prevention of mother-to-child transmission (pmtct) in Nelson Mandela Bay Health District: impact of a community dialogue intervention

- Description: Background: Globally, childhood mortality remains high with 5.3 million deaths occurring in children under the age of five years in 2018. Of these deaths, 43 000 were children from South Africa, with malnutrition being a significant risk factor contributing to disease and death. Breastfeeding has many benefits and reduces childhood mortality. Current primary health care strategies in South Africa are not having an adequate impact on breastfeeding rates, therefore appropriate messages need to be targeted at the community in order to improve breastfeeding rates. Participatory approaches have been identified as an effective tool to promote child health. Community dialogues led by skilled Early Childhood Development (ECD) mentors from the community might be key in order to improve caregivers’ knowledge, attitudes and practices (KAP) in relation to breastfeeding and Prevention of Mother to Child Transmission of HIV (PMTCT). Aim : The aim of the study was to explore the potential effects of community dialogues, led by ECD mentors, on caregivers’ knowledge, attitudes and practices (KAP) in relation to breastfeeding and prevention of mother-to-child transmission of HIV (PMTCT). Methods: Quasi-experimental design was used to compare the KAP of caregivers in relation to breastfeeding and PMTCT. Convenience sampling was used to include 63 caregivers who participated in the umbrella project (mixed-methods) and 61 caregivers who did not participate in the project. Ethics approval was obtained from the Nelson Mandela University Faculty Postgraduate Studies Committee. Participants had to provide written informed consent. A skilled interviewer used a structured questionnaire to assess the KAP of all participants. Data were captured on an Excel spreadsheet and analysed in collaboration with a statistician, who analysed the data using Microsoft Excel. Descriptive statistics were applied to numerical data, while frequency distributions were applied to categorical data. Correlations were calculated to describe the degree of relationship between two variables. Inferential statistic tests, conducted at a significance level of 0,05, were used for hypothesis testing. The t-test and analysis of variance (ANOVA) were used in tests involving mean values. Scheffé’s test was used in ANOVA to determine between which of the groups a statistically significant difference was found. Results: In the intervention group more than double the number participants (35%; n=22) had a good knowledge of breastfeeding when compared to participants (15%; n=9) in the control group. Additionally, the intervention group participants displayed better attitudes towards the RtHB (p=0.02). However, control group participants showed better knowledge towards the HIV (p=0.01). Finally, results suggest that in the intervention group, participants (33%; n=21) were more likely to have a good total KAP score in relation to breastfeeding and PMTCT when compared to participants (11%; n=7) in the control group. In the intervention group, better knowledge correlated with good practices relating to the RtHB (r=0.27). However, in the control group, positive attitudes correlated with better practices relating to HIV (r=0.29). Younger intervention group participants displayed better breastfeeding related practices (p=0.011). Additionally, younger control group participants displayed better knowledge of breastfeeding (p=0.001). Intervention group participants whose choices were influenced by their husband/ partner (p =0.016) and/ or radio/ magazines/ newspapers (p=0.004) had positive attitudes towards HIV. Control group participants whose choices were influenced by their other family members (p=0.043) and/ or clinic staff (p=0.016) had good knowledge of sharing their knowledge. Conclusions and recommendations: The intervention group displayed a better knowledge of breastfeeding as well as more desirable attitudes towards the RtHB, when compared to the control group. However, the control group displayed better knowledge of HIV, which could be due to the control group having more younger participants when compared to the intervention group. The community dialogue intervention had a positive impact on caregivers KAP relating to breastfeeding and PMTCT, which included all the different categories combined. In order to improve the Sakha Esethu intervention programme, managers need to provide guidance to the mentors on how to initiate conversations regarding HIV and discuss the stigma associated with HIV and breastfeeding. Results showed that including grandmothers and husbands or partners in the target population will benefit the intervention. Additionally, the use of radio needs to be considered especially for discussions regarding HIV. Co-generating a people-centred approach to addressing student hunger at a South African university

- Description: While considerable progress has been made regarding access to higher education in South Africa for previously excluded and disadvantaged students, great challenges are experienced in terms of student well-being. One of the key factors contributing to students’ ability to complete their studies successfully is food-related challenges. Various programmes have been introduced at institutions of higher education to assist and support students who lack funds for food. However, these programmes are characteristic of the welfarist model, as it is often based on needs, short-term by nature, and dependent on external resources. Furthermore, it reduces students to the role of passive beneficiaries. This research answers the question of how student hunger can be addressed through co-generating a people-centred approach at one higher education institution in South Africa. The study was underpinned by Sen’s capabilities approach to enable a theoretical understanding of the issue of student hunger. Additionally, Korten’s people-centred development provided the value base of the study to place students at the centre of the problem-solving process. An approach to student hunger that would be participative, student-centred, and enhancing nutritional capabilities was invited. Situated within the critical paradigm, I employed the participatory action research design to facilitate a process whereby students critically questioned the issue of hunger and took action to address the issue. Eight students from various study fields volunteered to co-research the issue of student hunger. Over the period of one year, we generated data using different methods (including photovoice, community self-survey, workshops, purposeful group discussions, focus group discussions, and my reflective notes). We applied cyclical processes to plan, implement, and reflect on actions intended to address student hunger. Through a combination of collaborative and individual processes of data analysis, we responded to four subquestions through four phases, which culminated in the co-construction of a people-centred approach to student hunger. Students’ conceptualisations of student hunger revealed two types: hunger of the stomach and hunger of the mind. The cyclical relationship between the two types infers that nutritional well-being is not achieved adequately when students have the means to acquire food. Choices regarding the utilisation of the available means also need consideration. The findings revealed that students apply the conscientisation framework in the ways that they plan to address student hunger. Applying value-driven processes, actions entailed physical dialogues with groups of people. The approach developed in this study offers strategies for the stakeholders who attend to student hunger, to collaborate and make contributions towards comprehensively resolving the complex issue. It primarily could be useful to institutions of higher education as the suggested coordinators of a collaborative structure. A second part of the developed approach puts forward a strategy to address the financial capabilities of students. The study contributes a consideration to constraints that stem from students’ mindsets and adoption of a prevailing culture that maintains student hunger. Engaging the female voice in understanding gender and sexuality interaction: the school and social work as medium for empowerment

- Description: The lack of the female voice in relationships with men means that they cannot negotiate safe sex, friendships with both male and female peers, their freedom of movement within the parameters of their relationships, etc. As a result of this, HIV and teenage pregnancy rates remain unacceptably high. The public health sector in present-day South Africa finds itself increasingly strained due to high levels of new incidents of HIV in young women between the age of 18 and 24. Statistics show that pregnancies are on the rise due to early sexual interactions. These statistics stand in stark contrast with the fact that gender and sexuality are topics taught in the school system through subjects such as Life Orientation. The assumption is often that young people, and in the context of this study, young women, will translate the knowledge into skill sets and behaviours that would help them assert themselves more and have control of their bodies. The statistics, however, seem to tell a different story. The purpose of the study is to understand why, despite the fact that there are subjects in the school curriculum and programs offered by different organisations, HIV infection amongst young women as well as teenage pregnancies remain a problem, an indication that the female voice remains marginalised in intimate spaces. The theoretical frameworks for this study include a Humanising Pedagogy, interrogating education and training engagements for social change while also engaging with various works of Judith Butler as a lens to view the marginalisation of young women in a gendered society. This study is qualitative in nature as it uses a critical ethnographic research design. It aimed to uncover and understand the deeper meanings of the phenomena experienced by young women from their point of view. The study employed a non-probability, purposive sample technique, in addition to the ethnographic analysis. A sample of 15 young women between the ages of 18 and 24 in the Nelson Mandela Bay Metro engaged in the study. These young women were recruited from Rise out of School Programme, part of the Khethimpilo prevention program. Informed consent to participate in the study was secured before interviews were conducted and participants were not coerced into participation and were free to drop out at any point. All endeavours to protect participants from physical harm and mental stress were taken. Misconceptions and misunderstandings which arose in the piloting phase of the study were removed and complete confidentiality was maintained in the management of all information obtained about participants. Individual semi-structured interviews were conducted with the participants. Thematic analysis was used to analyse data. Foundation Phase teachers working in low resource contexts, however, being generalists rather than specialists, struggle to teach PE effectively, even when supplied with programmes by external experts. I was interested in finding out why, and what could be done to help them integrate Fundamental Movement Skills (FMS) into their teaching, to benefit their learners. I proceeded from the hypothesis that teachers should play an active part in their own development, otherwise any Continual Professional Teacher Development (CPTD) would not bring about the change desired. I argued that if CPTD training and support is embedded within their context and based on teaching philosophies that foster teacher transformation, empowerment, and advocacy, then teachers might be more likely to commit to teaching PE. My aim in this study was thus to develop a collaborative process of CPTD with Foundation Phase teachers in low resource schools to enrich their pedagogical content knowledge (PCK) of PE and FMS within their contexts of social disadvantage. I initially adopted a qualitative design, but during this study, I realised that a more participatory approach was needed. I therefore present my study in two phases In Phase One, I answered the question: What are Foundation Phase teachers’ experiences of implementing PE in low resource schools? I did so to inform the design of an appropriately suited CPTD model for Phase Two of this study. I used a qualitative research design situated within the interpretive paradigm. I purposively sampled 24 Foundation Phase teachers to partake in semi-structured interviews. Thematic analysis revealed three broad themes, namely, personal, and systemic barriers to PE, as well as positive responses to challenges. The overlap between themes was significant, highlighting the complexity of teachers’ experiences and perceptions of PE. Teachers’ positive responses emphasised that they possess the creativity and advocacy to overcome challenges posed and necessitated that I adopt a participatory design to work with teachers to develop a collaborative form of CPTD. Phase Two of this study was guided by the questions set by the participating teachers. Ten Foundation Phase teachers from two schools volunteered to be part of the study. In Cycle One they asked: What do we need to learn to be able to effectively implement PE? The findings of Cycle One led them to ask the following question in Cycle Two: How can we improve our confidence to teach PE within our school contexts? Participants did so through collaboratively creating PE lessons which they then implemented and evaluated. The findings of Phase Two highlighted how Foundation Phase teachers can be guided to empower themselves to overcome the barriers to teaching PE that they face in their low resource schools. Based on teachers’ CPTD experiences, I then addressed the third question of this study: What CPTD guidelines can be generated to enable Foundation Phase teachers to improve on and to implement their PCK of PE and FMS at low resource schools? Five CPTD guidelines aimed at collaborative and transformative PE-based CPTD focused on whole school transformation were identified. I provide a graphic depiction of the CPTD guidelines that explains how it can be operationalised. Data was collected using a pre-test post-test format, administering the Child and Adolescent Mindfulness Measure (CAMM), the Five Facet Mindfulness Questionnaire (FFMQ-15), and the Emotional Regulation Questionnaire for Children and Adolescents (ERQ-CA). After the Mindfulness-based Adolescent Programme (MBAP), individual semi-structured interviews were used to explore the adolescent’s perceived experience of the programme. Data was analysed using Braun and Clarke’s (2006) method of thematic analysis for qualitative data. The researchers reflective journaling was also used to expand on findings. Themes that emerged include the participants reflecting awareness of change in perceived experience of mindfulness, growing emotional awareness and regulation, development of attentional abilities, development of metacognition, reduction in physical discomfort, building and enhancing relationships, and experiencing body-based practices as most beneficial. - Description: Despite the development in computerised systems and in planning and manufacturing programmes since 1950, Pharma-X* still record their manufacturing processes manually, using a paper batch record, (Manufacturing Batch Records (MBR) or Batch Manufacturing Records (MBR)) and equipment logbooks. There can be up to five logbooks and an equipment control label for each type of equipment used at each process step, which must be completed manually before manufacturing can commence. This paper-based system is a laborious practice, as excessive time is spent completing manual entries into the MBRs and logbooks. Lean thinking, a popular manufacturing improvement philosophy, would describe any such activity as non-value adding and therefore waste (Melton, 2005). Pharma-X started implementing a manufacturing execution system (MES) in June 2018 to automate the manual entries of the manufacturing process. Once the MES is completely implemented, it will allow pharmacists the opportunity to review the manufacturing batch in real-time and to conduct investigations when an error occurs. MES will also allow the operators to complete logbooks with a single scan of equipment, as well as decrease manual entries into the MBR. This study was a quantitative research study comparing data of the past state (Pharma-X pre-MES) with data of a current state (Pharma-X post-MES implementation) to assess the impact of the implementation of a manufacturing execution system and lean manufacturing on a pharmaceutical company. However, in a society where the technosystemic digital world has entered microsystemic parent-adolescent interactions and the developmental process of adolescents’ identity formation, it is crucial to understand how parents experience and perceive the role that social media plays in their adolescent children’s identity development and safety. The primary aim of this study is to understand parents’ perceptions and experiences of social media use in adolescent identity formation and safety. The researcher utilised a qualitative research approach with an exploratory descriptive design. A non-probability purposive sample was employed, and telephonic semi-structured interviews were conducted with 15 participants. All of the participants were parents of adolescents between the ages of 12 and 18. Interviews were conducted until data saturation was reached. The participants’ responses were analysed using thematic analysis. Four categories arose from the data analysis and demonstrate that social media was perceived to have both a helpful and unhelpful impact on adolescents’ identity formation and safety. The first category refers to the perceived role social media plays in adolescents’ development of physical, gender, sexual, and social identity. The permanence of social media, the role parent-adolescent relationships play in adolescents’ online identity experiences, and identity commitment and exploration maturity emerged as key themes. The second category of key findings associated with social media and adolescents’ safety include social media violations, the mental health of adolescents, emotional regulation, identity formation, and reality illusion as themes that emerged from the data. The third category encompasses the resources that parents perceive to be of assistance in maintaining and enhancing safe social media use among adolescents, namely parent-adolescent relationships, remaining educated, and adjusting monitoring in a developmentally appropriate manner. The fourth category refers to a network of resources that parents apply to maintain and increase safe social media use by their adolescent children. - Description: Cyberbullying is a significant field of study within cyberpsychology and requires continued research and development in order to further literature and understanding in a South African context. An increase in cyberbullying results in an increased prevalence of the occurrence of psychological distress in the form of short-term consequences as well as long-term chronic impacts such as depression, anxiety, and decreased life satisfaction. This qualitative study aimed to explore and describe the experiences of young adults who have experienced cyberbullying and the perceived impact of this experience on their psychological well-being. Qualitative research is a flexible exploratory and descriptive approach to understanding the lived experiences of an individual and their subjective view of a specific phenomenon through an interpretivist paradigm. This study utilised purposive and snowball sampling of Nelson Mandela University students aged 18 – 25 years old to gain a total of 6 participants for data gathering through semi-structured interviews. This research process followed a retrospective perspective which focuses on the specific meaning-making and understandings which that individual places on that experience, in relation to their psychological well-being. Thematic analysis was employed to analyse the data and trustworthiness criteria and was utilised throughout to ensure the credibility of the study. It was found that cyberbullying, through multiple forms and methods, has various short-term and long-term perceived impacts in several spheres. The perceived psychological impacts occurred in the short-term and were internalised to extend to long-term impacts on psychological well-being, of which the majority were negative. There are influencing factors to the development, coping and management of perceived long-term impacts which reflect the outcomes on well-being. The findings of this study highlight the perceived impact of cyberbullying on young adults’ psychological well-being which is vital for future recommendations and intervention. , Thesis (MA) -- Faculty of Health Sciences, 2021

- Description: Numerous studies have investigated the use of blended learning by academic staff at tertiary institutions. The purpose of this study was to examine the perceptions of blended learning by academic staff at the Faculty of Health Sciences at Nelson Mandela University. A related objective was to identify barriers and facilitators to the adoption of blended learning by academic staff. A sequential, exploratory mixed-methods design was adopted for this study whereby Phase 1 (focus groups) was used to inform Phase 2 (questionnaire) of the study. Results were analysed from both phases and organised according to strategies, support, and structure of a blended learning adoption framework. Existing support structures to assist with blended learning adoption, understanding of what blended learning is, confidence in using blended learning tools, and time to attend training were some of the findings of the study. The researcher concluded that academic staff in the Health Sciences Faculty at Nelson Mandela University are positioned in the second stage of the blended learning adoption framework, namely the early adoption stage of blended learning. Product-dust levels released from pharmaceutical manufacturing equipment and related health-based exposure risks

- Description: The study evaluated the product-dust levels that were released from manufacturing equipment during routine manufacturing of highly potent active pharmaceutical ingredients. The study was performed at a high containment manufacturing facility for Product X. These assessments are necessary to evaluate the level of containment performance in correlation with health-based exposure limits; and from this, to develop proper control strategies for the workplace. The equipment that was evaluated in this study included barrier systems (isolators, isolation boxes), dust-proof decanting systems (split valves), soft connections covered with continuous liners, and airtight intermediate bulk containers. Methodology The amount of uncontrolled product-dust that was released during the manufacturing process was quantified by establishing an air and surface monitoring programme. This was then compared to health-based exposure levels (HBEL’s) to ensure operator safety and to implement an effective control strategy. Air monitoring involves the sampling of airborne particles through filtration of the air with a filter using a sampling pump to draw a quantifiable volume of air through the filter. The filter is then tested in a laboratory which extracts and quantifies the amount of highly potent active pharmaceutical ingredient (HPAPI) on the filter. Surface monitoring is used to assess the level of product contamination on a surface. The technique uses a suitable adsorbent material to physically adsorb surface contaminants from the surface by wiping or swabbing. Protocols were designed based on a risk assessment that took the manufacturing process, literature, and facility design into consideration. Results The HBEL level of 0.2 μg/m3 that was calculated for the manufacturing process of Product X was found to be lower than the HBEL of 1.4 μg/m3; and consequently, the level of operator exposure was well within the acceptable occupational exposure limits. All surface monitoring results passed the acceptance criteria of less than 0.032 mg/100 cm2 except for excursions that occurred during breaches in containment. Conclusion The air monitoring programme facilitated the quantification of the product-dust levels that were released into the primary manufacturing areas; which could then be verified against the HBEL of Product X. The surface monitoring programme facilitated the quantification of the product residue on non-product contact surfaces in the facility; which could then be verified against the HBEL of Product X. The objectives of the study were met for Level 2 of the high containment suite as it was possible to calculate the product-dust released from the manufacturing equipment and the product residue on external equipment surfaces and non-product contact surfaces in order to verify these levels against the relevant HBEL’s.
